Introducción: El rol de la profilaxis anticomicial en el paciente con un hematoma subdural aislado aún sigue siendo incierto. Objetivo: Evaluar la prevalencia del uso de profilaxis farmacológica con agentes anticomiciales en pacientes con hematoma subdural aislado y su asociación con la aparición de episodios epilépticos desde el diagnóstico y hasta los 6 meses de seguimiento. Pacientes y Métodos: Se realizó un estudio observacional retrospectivo que incluyó pacientes con un hematoma subdural como única lesión, internados en terapia intensiva. Se evaluó el uso de la profilaxis anticomicial al alta y se registraron los episodios convulsivos durante un seguimiento de 6 meses. Resultados: Se incluyó y analizó a 55 pacientes dados de alta del Servicio con diagnóstico de hematoma subdural en un período de 5 años. El motivo de consulta más frecuente fue el traumatismo craneoencefálico (44%), seguido de déficit focal neurológico nuevo y cefalea (22% cada uno). El tipo de hematoma más frecuente fue el agudo (67%) y, en el 84%, la causa era un antecedente reciente de traumatismo. Ocho de los pacientes (15%) recibieron un fármaco anticomicial como profilaxis al alta hospitalaria. Durante el seguimiento, un paciente (2%) tuvo un episodio convulsivo, mientras estaba recibiendo profilaxis anticomicial. Conclusión: Los episodios comiciales luego de un hematoma subdural son fenómenos poco frecuentes y el rol de los fármacos anticomiciales para su prevención es aún incierto.
