Arterial vein ECMO as support of cardiovascular function in the postoperative state of the child with a hypoplastic left heart

Summary
Introduction: Hypoplastic Left Heart Syndrome (HICHS) is defined as a serious birth defect characterized by underdevelopment of the left heart. Low cardiac output syndrome that is refractory to vasoactive drugs during the postoperative period can be an indication of venoarterial extracorporeal membrane oxygenation (ECMO).
Case: A child with HICHS and refractory inotropic support after Norwood Surgery was initiated on ECMO. She remained on ECMO during 4 days allowing the systemic ventricle to recover. She was extubated on the 22nd postoperative day and discharged from hospital 2 weeks later.
Discussion: ECMO in patients with univentricular physiology has been considered a contraindication. In this case without residual defects, ECMO support allowed time for the heart to recover.
