Trombosis venosa cerebral e hipertensión endocraneana

Abstract

La trombosis venosa cerebral (TVC) es una forma poco frecuente de accidente cerebrovascular con una baja tasa de mortalidad. A partir de un caso clínico de una paciente con TVC e hipertensión endocraneana severa, realizamos una revisión a través de una búsqueda bibliográfica acerca de las características de esta entidad haciendo especial hincapié en las opciones terapéuticas. La sintomatología de esta patología varía desde cefalea aislada hasta el coma, y para el diagnóstico contamos con TC o RMN asociado a venografía cerebral. Se recomienda fuertemente la anticoagulación, mientras que la trombolisis y la craniectomía descompresiva serían una opción en grupos con mala evolución. La falta de evidencia sólida, pone al médico tratante en la difícil tarea de decidir qué camino tomar ante un paciente con este diagnóstico. El monitoreo neurológico y la prevención de la trombosis conforman los pilares para obtener una evolución neurológica favorable.
