High-risk non-ST-elevation acute coronary syndrome as a severe clinical manifestation of Cogan's Syndrome

Abstract

Cogan's syndrome is a rare inflammatory pathology that usually presents with ocular and vestibulocochlear symptoms, but in a low percentage of patients it can be associated with systemic manifestations vasculitis-like. The treatment of this syndrome is not clear and is based on immunosuppressive regimens similar to those of other autoimmune diseases. When it presents with severe manifestations such as acute coronary syndrome with a possible indication for surgery, best time for the invasive procedure is unkown. We present the case of a patient with Cogan's syndrome and aortitis that caused significant extrinsic compression of the left main coronary artery, as well as its evolution, treatment, and review of the currently available literature.
