Portal vein gas due to intestinal ischemia secondary to superior mesenteric artery thrombosis

Abstract

75-year-old patient, with abdominal pain of 12 hours of evolution accompanied by general malaise. Severe hypotension and poor distal perfusion with impaired level of consciousness was observed. Laboratory tests revealed severe lactic acidosis (pH 7.04; pCO2 55 mmHg; HCO3 15 mmol/L; Base Excess -15.5 mmol/L, lactate 161 mg/dL)

Orotracheal intubation and stabilization were performed, and abdominal Computed Tomography was performed. Massive intestinal and portal pneumatosis (image 1, red circle) secondary to intestinal ischemia caused by thrombosis of the superior mesenteric artery (image 2, red arrow) was observed.
